NEW REQUIRED MEMBER WAIVER
The Fond du Lac Family YMCA has updated an agreement we require for our members. Our new agreement includes updated waiver and liability language regarding the potential to contract COVID-19 while visiting the facility.
Beginning October 1st, please review the new agreement and acknowledge it by signing.
Sign the waiver in the MEMBERSHIP AGREEMENTS PORTAL below using your member barcode found on your key card. You may also sign the waiver in these additional ways:

Via a link in a member email scheduled September 30th, sent to members having a valid email address on file.

By scanning the QR code using your mobile camera, which will be available on signage in the Y lobby, beginning October 1st.

At the Welcome Center desk beginning October 1st.
Beginning October 1st, you will not be able to check in at our facility until we have obtained a signature.
The Membership Agreements Portal will have the new agreement available to sign beginning October 1st.
